Application Form for Spay & Neuter

Our program includes your cat's spay/neuter, their core vaccines, a dose of limited dewormer, pain meds (both a shot and to take home) and either an ID tattoo or a microchip depending on which of our trusted veterinary partners we work with on the day of your cat's surgery.

We do not ask for proof of income, but rely on the honesty and compassion of those in our community accessing our program - we service many seniors, students, single parents and people on income assistance and disability.

If you are in a position to give a donation at any point in the future, or an ongoing donation please let us know, as those funds are much appreciated, and will help spay/neuter underprivileged cats in our city. We respond to all applications within 2-3 business days.

I give consent for Ena’s Community Cats to bring my pet to one of our trusted veterinary partners for a spay/neuter procedure. I understand that my cat will be undergoing a surgical procedure, and understand that there may be risks to this procedure and anesthetic episodes.

Ena’s Community Cats, their veterinarian(s) and staff are to use all reasonable precautions against injury, escape or destruction of the animal, but will not be held liable or responsible in any matter whatsoever, in any circumstances, on account of treatment, care or safekeeping of the animal, or otherwise in connection therewith, as it is thoroughly understood that I assume all risks.

In case the animal(s) are not picked up on time, a written notice will be emailed to the address on file to remove the animal(s).

Five days after such written notice the animal(s) will be considered abandoned and will be surrendered to a local rescue agency at Ena’s Community Cats’s discretion.

I hereby certify that I am 19 years of age or older, I have read and fully understand the above conditions of this hospital and acknowledge a copy of this form can be provided to me at any time by request.
